CMS posted the updated Quality Measures manual – version 12.1 – in early September 2019 then pulled it off the website a short time later. The updated QM manual – still version 12.1 was posted again on October 8, 2019 in the form of a .zip file this time. In that .zip file, you’ll see these documents:
